GRMC Home Health

GRMC Home Health is a full Service Agency that provides care to individuals in the comfort and privacy of their homes.  Due to advances in technology and training, out professional staff can offer patients and their families affordable care and education in a psychologically pleaseing environment.

Under a physician's direction a multifaceted treatment plan is developed.  This plan provides for a patient's physical and emotional needs and offers optimum support for recovery.  This multi-disciplinary approach is coordinated by the patient's GRMC Home Care Registered Nurse.  The RN coordinates a team of skilled, caring professionals who provide the health care and services which allows the patient to remain at home.

Services

    Skilled Nursing Care - A Registered Nurse (RN) asseses the patient's needs and with the patients physician, designs and individual plan of care.  The RN coordinates additional personnel that may be needed to meet patient and family needs.

    Certified Nursing Assistants - Experienced Certified Nursing Assistants aid patients in maintaining adequate personal hygiene and the performance of essential day-to-day activities.

    Physical Therapy - Physical Therapists perform exercise treatments to help patients regain strength, coordination and help relieve pain.  Instruction will be provided for the use of prosthesis, orthotic devices and assistance devices such as wheelchairs, walkers and canes.

    Speech Therapy - Speech Therapists work with patients to improve speech, hearing, language and swallowing.

    Occupational Therapy - The Occupational Therapists work to help patients improve environmental safety needs, cognition skills, coordination, sensory deficits, activities of daily living and the process of splinting and positioning upper extremeties.

    Medical Social Services - Skilled professionals assist the patient and their families with identifying community services that contribute to a higher quality of life.

    Mental Home Health Services - Registered Nurses skilled in acute psychiatric care provide an alternative to hospitalization.  Individualized plans assist patients with the transition from hospital to community, which can reduce relapses and readmission to the least restrictive environment.

    Additional Professional Services - Medical Supplies, Home Administration of I.V. Therapy, Chemotherapy and Enterostomal Therapy.

Payment Options

    Medicare - GRMC Home Health Care is a Medicare certified agency.  Certain Criteria, outlined in the Medicare guidelines, must be met for medicare insurance coverage.  These include, but are not limited to the following:

            The patient must be homebound, not necessarily confined to a bed.  A patient may leave home; however, it must require a considerable taxing effort.  These absences from home must be infrequent and of short duration.  Absences from home to receive medical treatment may include outpatient kidney dialysis, outpatient chemotherapy and outpatient radiation therapy.  Patient's residence may be a house, an apartment, a relative's house, a home for the elderly or any other institution considered to be the patient's residence.  Hospitals, skilled nursing and intermediate care facilities do not qualify under medicare guidelines as places of residence.  The patient must be under the care of a physician who has approved a plan of care indicating types of services required by the patient, their frequency and duration of these services.  This plan will be reviewed by the patient's physician every 60 days.

The skilled services provided include speech therapy, occupational therapy and skilled nursing on an intermittent, reasonable and necessary basis.  Patient condition, complexity determining whether services requiring nursng skills are reasonable and necessary.

Services for skilled observation and assesment are covered for a limited time under medicare when the likelihood of change in a patient's condition requires nursing skill to evaluate the need to modify treatment.

Other payment options include Medicaid, Champus, Commercial Insurance, Provate Pay and Worker's Compensation.
